If you are interested in using the CAHS software, please contact us.
For those who have signed a CAHS End User License Agreement, the contact person for your entity should download the following:
1. Entity Request Form
Please complete one form for your organization to be added to the CAHS application. (Entity Request Form)
2. User Request Form
Please complete one form for each person in your organization who has completed training in the DEMO application and needs access to the live/production CAHS system. (User Request Form)
3. Change Form
Please complete one form for all communications that do not involve setting up a user or entity, for example: changing the Contact Person information, changing the status of a user, or updating user contact information. (Change Form)
Once training is complete, the following forms may be downloaded for your internal use. These forms will help you facilitate the process of entering data into the CAHS system.
